An 18-month investigation by the House Transportation and Infrastructure Committee said faulty assumptions, a lack of transparency and insufficient oversight by the Federal Aviation Administration led to two Boeing 737 Max crashes that killed 346 in Indonesia and Ethiopia in 2018 and 2019.
